How to Apply for Cannabis Certification

Go to medicalcannabis.utah.gov and scroll down to Electronic Verification system box then click on that.

Go to “create Utah id” and create your ID

Read all info on the medical cannabis website

Keep your sign-in info

Make appt with doctor with "book appointment" link above

Bring the last 4 digits of your SSN and Drivers license with you

Pay for your doctor visit

Pay the state for your card

You are ready to go to the dispensary
